The Initial Evaluation
At Central Physical Therapy each patient is given a comprehensive 30-60 minute initial evaluation.
During this evaluation you will get a chance to tell us about your symptoms and their effect on your daily life, as well as any treatments you have received to date.
We will also obtain detailed medical, surgical, family, and social history from you.
Please arrive at least 15 minutes before your appointment to complete the required paperwork. You may also download and complete the forms ahead of time and bring them on your first visit, or fax and email them to us.
Please bring a referral for physical therapy from your doctor, your insurance card, and any medical reports pertaining to your condition (if available).
Please wear or bring with you comfortable clothing, such as loose shorts, T-shirts, or tank-top which would facilitate physical examination and treatment.
*If you currently have a deductible with your insurance for Physical Therapy you will be responsible for the full cost of the following:
