Initial visit could be about 60-90 minutes. Please arrive at least 15 minutes before your appointment to complete the required paperwork. You may also download and complete the forms ahead of time and bring them on your first visit.
Please bring a prescription from your doctor, your insurance card, and any medical reports pertaining to your condition (if available).
Please wear or bring with you comfortable clothing, such as loose shorts, T-shirts, or tank-top which would facilitate physical examination and treatment.
Hands-on treatments generally require at least 30 minutes in the private treatment room and 20-30 minutes exercise program in the exercise area. Treatment plans vary depending on your needs
Yes, effective communication is essential to any successful treatment. If you do not understand something about your treatment plan, please let us know right away.
Yes. Your doctor will get the initial evaluation report and discharge summary. In addition, we will communicate with your doctor if there are any issues that need clarification.
A referral is an authorization from your Primary Care Physician (PCP) yo see an in-network specialist. Most HMO’s require a referral. A prescription is the written order for physical therapy from the referring provider. Please check with your insurance carrier to find out if you need a referral or if your physical therapy prescription will be enough.
It is your responsibility to obtain a referral from your physician. If a referral is required, but not received by our office at the time of your first visit with us, you need to sign a form acknowledging this.
It depends on your insurance plan, you may be responsible for a co-payment and/or deductible to us at the time of your office visit.
